2011/12 Full-Time Bursary/Award Application Undergraduate and Graduate Students:

Full-time Ontario and out-of-province students may use this form to apply for a University of Waterloo bursary. Note: Form is available the beginning of the 2nd month of each term until the deadline date.
Waterloo International Full-Time Undergraduate Emergency Bursary International Students:

International students should use this form to apply for a University of Waterloo Full-Time Undergraduate International Emergency Bursary.
Part-Time Undergraduate Bursary Application Part Time Students:

Use this application to apply for a bursary if your normal course of study is part time (less than 60% course load).

Continuation of Interest-Free Status Form Complete this form and submit it before the end of each non-funded study term or official co-op work term, to maintain interest-free status on your previous OSAP loans. Please write your student ID number on the form and send the original form to the Student Awards and Financial Aid office; no faxes please.
Request for OSAP Access Number - OAN Request your OSAP Access Number (OAN) if you have forgotten it.
OSAP Application Update Form Complete this form to change information on your current 2011-2012 OSAP application.
Mail out Request Forms Students who will be studying: on exchange within Canada and the USA, via distance education, Architecture students, and Optomertry students on externships.
OSAP Rollover (Consecutive term) Complete this form to add a consecutive term to your OSAP assessment for the current academic year. For example, to add the winter term to your fall OSAP assessment. Note: Form is available the beginning of the 2nd month of each term until the deadline date.

OSAP Reinstatement Form (Non-consecutive term)

Complete this form to add a non-consecutive term to your OSAP assessment for the current academic year when you have experienced a gap of 4 weeks or more in your studies. For example, if you have a co-op work term in the winter, and wish to add the spring term to your fall OSAP assessment, or if you withdrew early from the fall term and wish to add the winter term. Note: Form is available the beginning of the 2nd month of each term until the deadline date.
Studies at Another Institution Form Complete this form if you are enrolled in courses at another post-secondary institution while attending the University of Waterloo to request costs from the other institution be added to the University of Waterloo costs for the applicable OSAP assessment.
Change in status to married or common-law/same sex relationship n/a

Adjust Local Travel Review

If you are a single student living at home during your study period and using a vehicle to commute to school as your home is more than 40 km from UW, or it takes more than one hour by public transit and/or public transit is not available you should complete this review for additional local travel costs.  Married students or sole-support parents whose home is more than 40 km from UW, or more than one hour by public transit and/or where public transit is not available are also eligible for consideration.

Adjust Income Contribution Review Complete this form to appeal your expected income contributions in the pre-study period and from a student's spouse in the pre-study and study period (if applicable). Note: we cannot process these reviews before the start of the term.
Adjust Living Allowance Review

Complete this form if:

  1. You are not living with your parents during the study period because their home is within 40 KM of the University but there is no public transit to UW or it is more than one hour away by public transit
  2. You did not live with your parents during your pre-study period because you were working full-time in another city
  3. You incurred and paid eligible exceptional expenses during your pre-study period.

OSAP Review for Assessment on Parents/Spouse Lower Estimated Income Complete this form if your parent's or spouse's income for 2011 is expected to be lower than their reported income for 2010 due to a non-recurring and major change.
OSAP Family Breakdown Review Complete this form if you have had a final rift with one or both parent(s)/step-parent(s) that prevents you from having contact with them to complete your OSAP application.
OSAP Residency Review Complete this form if you/your family have lived in Canada for less than 12 months, and Ontario has been the only province where you resided. You must also complete the appropriate residency history form below to document your residency. All forms must be completely filled in, and include original signatures.
Dependent Students: Use the History of Canadian Residency for Students and Parents to document you and your parent's residency since first arriving in Canada (attach an extra sheet if necessary).
Married Students: Use the History of Canadian Residency for Students and Spouse to document you and your spouse's residency since first arriving in Canada (attach an extra sheet if necessary).
Independent Students: Use the History of Canadian Residency for Students to document your residency since first arriving in Canada (attach an extra sheet if necessary).
